When a child is recovering from Long COVID, new issues can appear that feel confusing and scary. These aren’t random problems; they’re often secondary conditions that emerge because the original illness placed significant stress on the body—especially the nervous system. Seeing the whole picture helps families focus on healing the underlying disruption, not just collecting new labels.

Evidence in pediatric cohorts indicates that post-COVID autonomic nervous system dysregulation may be associated with the development of POTS and other forms of dysautonomia.

Can Long COVID cause POTS?

In practical terms: yes—Long COVID can be associated with POTS in children and teens.

POTS has been increasingly reported in pediatric populations post-COVID. Studies indicate that the viral infection may trigger dysregulation of the autonomic nervous system, leading to POTS and other dysautonomias in children and adolescents (7).

Research published in Frontiers in Pediatrics highlights that pediatric patients with Long COVID frequently present with symptoms overlapping with POTS, including orthostatic intolerance, exercise intolerance, and severe fatigue, which can significantly impair quality of life and school attendance (8). The pathophysiology is thought to involve immune-mediated damage, persistent inflammation, and possibly viral persistence.

POTS is one of the secondary conditions that can emerge alongside or after Long COVID in this age group.

Why Early Recognition Helps

Identifying POTS in the context of Long COVID allows families and clinicians to align around supportive, targeted measures aimed at function and quality of life. Helpful, individualized strategies may include increasing fluids and salt, carefully graded activity, and, when appropriate, medications—all within a coordinated, multidisciplinary approach as care evolves.

When to Consider a POTS-Focused Evaluation

Ask about POTS if your child has two or more of the following—especially after COVID:

Difficulty returning to school/activities due to orthostatic symptoms
